The CLEAT Benefit Plan is an ERISA Plan which provides dental and/or vision benefits to participants of the plan. Only members of a covered unit may join the CLEAT Benefit Plan. A Covered Unit shall mean a group of Members organized as any one of the
following:

• employees of the Administrator
• members and employees of a collective bargaining unit
• employees of a political subdivision

Each Covered Unit shall execute an Adoption Agreement. An Adoption Agreement shall mean that binding legal agreement entered into between the Administrator and a Covered Unit whereby the Covered Unit adopts the Plan and those certain Schedules of Benefits which it has selected.
